Default Built-in Form Fields multiple copies after upgrade

Total of (5) copies each for "First name", "Last name", and others are listed in the Tools>Form Fields page in the admin module for both Account Signup and Address tabs.

Problem: Cannot delete the extra (4) for each field name as they are of status "built-in"

Solution: Not sure why the upgrade had a hiccup, but the extra (4) copies of the field can be deleted manually from the isc_formfields table.

Always backup your data before any modification or upgrade!

a client of mine upgraded his copy to the latest build last night and I noticed that all the custom fields that we had made for each of his products have now disappeared. they appear when we goto the product edit page and click on custom field tab but on the main store for the product page they are not appearing.

any idea how to fix that?

Default Needed Files For Upgrading from 5.0.2 to 5.05

I don't want to overwrite all of the files on the server.
What do I need to get the new changes to work and the bug fixes?

Also the post mentioned something about removing the the top and
bottom promotion panels. Does this mean that the banners will
not be there?

I'm assuming that this upgrade will kill the accordion menu. and all
other modifications I made, language.ini etc.

First thing - make sure your entire site is backed up along with a copy of your database. You shouldn't overwrite all the files because you will may lose your modification/changes. It's better not to risk it. You've got to do a comparison on the changed files and carefully merge any files you changed that were changed in the release. The others you should just be able to overwrite. That's the only way. It is a very cumbersome and tedious process but if you want it done right its going to take time. The last couple upgrades have taken me around 2-3 hours each.

http://winmerge.org/ is one tool for windows that will help you in this process. Another thing you might want to do is check your existing site into a version control system. That's what I've done and it's great because if I ever need to revert to a previous version it's easy.
