When I go to the create category page I see an error "An error occured while trying to load category icons"

This is usually because the Knowledgebase url on the settings page is incorrect. Double check that this is right.

If it is the right url then, if you are using mod_rewrite for your url rewriting, you probably have to specify the RewriteBase in your .htaccess file. See the related questions for how to do this.
